Parent Toolkit


ACH is the preferred billing method for Spark Learning LLC.  All payments will be scheduled to be deducted from your account on a bi-weekly schedule (see below) regardless of your enrollment type.  Example:  If you enrolled for several drop in weeks, your payment will deduct from your account according to those time frames.

A $20.00 fee will be assessed to your account if Spark receives notification of a return payment due to insufficient funds or unauthorized payment from your financial institution.  If Spark receives two (2) returned payments due to insufficient funds, invalid or closed account notifications, or unauthorized payments, within one summer, your child may be removed from the program.   Please refer to the Family Policy for further details on payment policy (scroll down on this page to find the link to the family policy)

Payment Schedule:

  • May 31 withdrawal covers time at Spark from June 3 - June 14
  • June 14 withdrawal covers time at Spark from June 17 - June 28
  • June 28 withdrawal covers time at Spark from July 1 - July 12
  • July 12 withdrawal covers time at Spark from July 15 - July 26
  • July 26 withdrawal covers time at Spark from July 29 - August 2 (note this is only one week)

Forms

  • This form can be used by all parents to notify Spark of a planned absence from the program.

    Session 1: During our 10 week program, we require a 9 week minimum. Please use this form to indicate to us what week, if any, your child plans to be gone from Spark Summer Learning. You will not be charged for that week that they are not there.

    Sessions 2 & 3: Please use this form to notify Spark of a planned absence for your child during their enrollment period. You will be charged for any missed days as all 4 weeks are required for these enrollment options.

    Drop-ins: Please use this form to notify Spark of a planned absence for your child during the week of their enrollment period. You will be charged for any missed days as the entire week is required for this enrollment option.

    Note: Spark will be closed on July 4th for the holiday.

  • If you have any information that has changed, please fill out this form. This is a web based form that will be electronically submitted.

  • This form allows you to withdrawal your child from Spark Summer Learning. Please discuss with the Site Director before submitting this form. This is a web based form that will be electronically submitted.

  • Please view the DHHS Parent Brochure. You will be required to sign the DHHS Parent Brochure Acknowledgement form below once the review is completed.
